Need some help Im so computer illiterate..

Hello, I am trying to make a certificate of authenticity using Microsoft WORD.

So far I am not doing very good and have wasted a huge amount of time and efforts. Any of you Dogs with some experiance on this that would be willing to help a old Dog out shoot me a message.
